Holotype: BPBM 16883, 51.5 mm SL, male.

Type locality: patch reef on north side off Gannet Ridge , Pitcairn Island, depth 40 to 44 meters. Illustrations: Kuiter, 2004:64, figs. A & B; Randall, 1979:7, fig. 7.
