Holotype: BPBM 10560, 76.9 mm SL, male.

Type locality: Moku Manu (off north side at base of drop-off), Oahu, Hawaiian Islands, depth 49 meters. Illustrations: Kuiter, 2004:63, figs. A–E; Randall, 1979:7, figs. 5 & 6.

D: X, 17 (rarely X, 16; X, 18; or XI, 18). A: III, 9 (rarely 10). P: 15 (rarely 16). C: 15 (8 + 7). V: 26 (10 + 16). S: 2. GR: 7 to 9 + 22 to 25. LL: 40 to 46. CP: 22.

Distribution: central Pacific: Hawaiian Islands and Johnston Island.
