Holotype: BPBM 10560, 76.9 mm SL, male.
Type locality: Moku Manu (off north side at base of drop-off), Oahu, Hawaiian Islands, depth 49 meters. Illustrations: Kuiter, 2004:63, figs. A–E; Randall, 1979:7, figs. 5 & 6.

Distribution: central Pacific: Hawaiian Islands and Johnston Island.
